Home Health — skilled, medical care after a hospital stay or new diagnosis
Palliative Care — relief from symptoms and stress, at any stage of serious illness
Hospice — comfort and dignity in life’s final chapter
For our aging population, HCBS means one thing above all: staying home, staying connected, and staying in control of how they live — and how they’re cared for.
